Appointments

To make an appointment, please call our office at (804) 433-1400 during regular business hours.

Please make sure to bring a list of all medications and a copy of your insurance card to each visit. If there have been any changes to your personal information or if it has been a year or more since you have been seen, you may be asked to fill out a new registration form.

Currently our office hours are as follows:

Monday 8:00 am - 4:30 pm
Tuesday 8:00 am - 4:30 pm
Wednesday 8:00 am - 4:30 pm
Thursday 8:00 am - 4:30 pm
Friday 8:00 am - 4:30 pm

You can reach our office by telephone at (804) 433-1400 during regular business hours. For serious medical issues arising outside of our regular business hours, you can reach our answering service by dialing (804) 257-5177.

New Patients

New patients should arrive at least 15 minutes early for registration. To help with the registration process, you may download and fill out our patient registration forms and bring them with you for your appointment.

Cancellations

If you arrive late for your appointment, you may be asked to reschedule. This helps to ensure that each scheduled patient can be seen in a timely fashion. We request 24 hours notice on all appointments that need to be cancelled.
